I understand that this procedure is a permanent change to my skin and body. If at any point in the future I am unhappy with my tattoo, I understand that I may have the tattoo covered or removed.

I 100% allow my tattoo to be photographed and used in the Tattoo Shop portfolio, as well as in promotions online and in-person.

If for any reason myself or the artist does not end up finishing my tattoo, I agree that it will be fully covered by the artist of my choosing. Beyond Belief Tattoos custom work is not to be finished by anyone else.

I acknowledge that BBT does not offer refunds for gift cards, deposits, full payments, or tips.

I agree that BBT does not have a way of identifying whether I am allergic to the elements or ingredients that will be used during my tattoo process.

I acknowledge that BBT emailed me aftercare instructions. I agree that I will follow those instructions to ensure my tattoo heals properly. I understand that I need to take care of my tattoo by following the instructions provided to me by the Tattoo Shop.
